办理口罩出口通关单的流程及所需资料（此资料仅供参考，如果与海关的要求有异议，以当地海关为准）
申请地：各个工厂所属的管辖海关。比如：工厂所属地在广州的白云区，就在广州海关申办，工厂所属地在佛山海关，就向佛山海关申办，只能属地申报
申办的资质要求：
1. 工厂需要有卫生证
2. 工厂需要在所属地海关办理海关商检备案
3. ***口罩需要***监督局的备案。

申办所需提交资料：
1。报检委托书（有专门的格式）
2.出口声明（有专门的格式）
3.合同，***，装箱单（出口报关专用）
4.厂检单（工厂自己的检验合格的凭证，无格式要求）
5.提交样品（是否需要提供样品去实验室进行检测，目前暂时不确定）
