一、 需提供的材料：
(一)《进出口宠物食品标签审核申请书》：按要求填写完整的《进出口宠物食品标签审核申请书》
，标明申请日期、申请人签字并盖公章。
(二)宠物食品标签样张共8份。
(三)进口宠物食品在生产国(地区)允许生产及销售的证明文件。
(四)生产商和经销单位的营业执照(复印件)，外资企业需提供外企登记证(复印件)
(五)检验检疫机构出具的检测报告。
(六)如标签中特别强调某一内容(如皇家特级、销售经久不衰、法定产区、珍藏版、时，须提交证明材料。
(七)如标签上标注经高标准或ISO9002认证，则需提供相关的证明材料。
(八)如标签上标注曾获得某些荣誉或奖牌，则需提供相关的证明材料。
(九)***食品则需提交***批准的***批文(复印件)。
(十)检验检疫机构根据审核需要要求申请人提供的其他有关材料。
(十一)所有的审核上报材料，凡有外文资料的均应提供相应的中文翻译件，并加盖申请单位的公章。
1、 Materials to be provided:
（1） "Application form for inspection of import and export pet food labels": complete the application form for approval of import and export pet food labels as required
, indicating the date of application, the applicant's signature and official seal.
（2） There are 8 samples of pet food label.
（3） The certification documents for the production and sale of imported pet food in the producing country (region).
（4） The business license (copy) of the manufacturer and the distribution unit, and the foreign-funded enterprise shall provide the registration certificate (copy) of the foreign-funded enterprise
（5） Inspection report issued by inspection and quarantine institution.
（6） If there is a special emphasis on a certain content in the label (such as Royal special grade, long-standing sales, legal production area, collection version), supporting materials must be submitted.
（7） If the label is marked with high standard or ISO9002 certification, relevant supporting materials shall be provided.
（8） If the label indicates that some honors or medals h***e been won, relevant supporting materials shall be provided.
（9） Health food shall submit the Health approval (copy) approved by the Ministry of health.
（10） Other relevant materials required by the inspection and quarantine institution to be provided by the applicant according to the needs of the examination.
（11） All audit and report materials with foreign language materials shall be provided with corresponding Chinese translation and stamped with the official seal of the applicant.
